Beginning the Process:

• Admission to Sobriety House begins with a pre-screen assessment with our Intake Coordinator. This assessment is completed over the phone and takes 15-20 minutes. Admissions are accepted on a first come first serve basis, except for our priority populations.

• The screening process will determine eligibility for priority placement, fitness for our program and eligibility for grant funding.

• Priority placement is given for certain clients that fall into the state defined categories, such as women who are pregnant and women with dependent children, IV drug users, Involuntarily committed clients, and Veterans. 

• If we are unable to immediately schedule an intake date for you, we will place you on our waitlist. Do not be discouraged-we will provide you with resources while you wait and, if necessary, we will refer you to another program. We ask clients to follow the instructions of the Intake Coordinator and call in once a week to stay on the list.

When you Call:

Please have the following information ready to share and be in a safe, private location when you call.

• Substance use concerns, including the last date of use.

• Substance use history.

• Psychiatric history and diagnoses.  

• Medical history, including any medications you are currently taking.

• Legal issues (past and current).

• Past treatment history.

• Medicaid ID, Insurance information and/or Income and copy of W2 if applicable.

This information will be reviewed by our staff in order to determine clinical appropriateness for our program.

Eligibility:

• Willingness to participate in program activities.

• Medically safe to enter treatment.

• Sobriety House is required to follow Denver City ordinances that place certain restrictions on those that can reside on our properties. The pre-screen assessment will address these issues.

• An intake appointment does not guarantee admission. Final determination of acceptance is made clinically after a formal assessment on the date of intake. 

• Sobriety House is not a medical, detox or dual diagnosis facility. If your needs are determined to be outside the scope of the agency’s expertise, staff will work with you on referrals to more appropriate facilities for your needs.  

*Sobriety House is committed to helping those most in need find treatment. We offer priority placement to pregnant women, women with dependent children, IV drug users, and involuntarily committed clients. We also offer priority placement to all Veterans of our armed forces. Veterans may be eligible for funding through a special grant. 

Family and Friends:

Substance Use Disorder impacts the family and close relations of those that suffer from it almost as much as it does the sufferer. Sobriety House recognizes this fact and we strive to help prospective clients’ friends and family understand and deal with this reality. If you are concerned about your loved one, you are welcome to contact our Intake Coordinator with questions about our program and discuss options for care that we have available. We can also assist with references to detoxification facilities and other treatment programs that may be more clinically or medically appropriate. 

Confidentiality and Patient Rights:

All calls to Sobriety House are confidential. We will never share your information with anyone without your express permission. If a friend or family member is the first call, once we speak to a prospective client all communication will be with that client from then on-unless the client gives us permission to talk to friends or family through a written release of information. 

Our commitment to each patient’s confidentiality is ensured by our legal responsibility as mandated by state and federal law. Each member of the Sobriety House team is dedicated to upholding these standards in all communication and records. You will be asked to complete releases of information if you wish Sobriety House to provide or obtain protected health information to any individual or organization. 

At Sobriety House, we place a premium on providing respectful treatment of patients, family members, and staff in a safe and supportive environment. The privacy rights of our clients and their family members are preserved and protected. These rights are outlined in the client handbook that will be given to you upon admission.
